MAAC PRE-REFERRAL FORM

Dear MAAC Friends,

In order to seamlessly and effectively serve our MAAC youth, the following new guidelines are now in effect starting 01-OCT-2018 for the registration of youth with our MathPlus program:

(1) Please fill the PRE-REFERRAL form on this page, so that we can contact the home, introduce our group and verify availability of schedule for both the student and our current Master Instructors. This process may take up to a week, depending on the promptness of response of the Home Decision Maker. PLEASE NOTE: THE PHYSICAL ADDRESS IS REQUIRED HERE!

(2) Please note that the student will be placed on a waiting list, and you will be advised of the position of the student within the list. Additionally, WE REQUIRE ONE FORM PER YOUTH, REGARDLESS OF THE FACT THAT MORE THAN ONE YOUTH MAY RESIDE IN THE SAME HOME!

(3) If availability is confirmed, you will receive an email from your group with a copy of the audio from the call, as well as a link to complete the intake process.

(4) Once this information is received, we will register the youth in our database and we expect to have the FIRST session scheduled in 3 business days or less. If there are any issues, we will reach out to you and the MAAC, keep the youth on the waiting list and resolve the matter as quickly as possible.
